APHIS Data Transfer Permission Form

Obtain permission for a client's APHIS animal records to import into VetIMPRESS. 

APHIS Data Transfer Permission (Prefilled) Form

Permission must be registered with DAERA via an agent authorisation form (AAF2) before a farmer’s data can be brought into your VetIMPRESS site. The farmer's holding must also be set up in VetIMPRESS, with Holding Number and Customer ID entered as a minimum. Animals can only be imported if the Holding Number and Customer ID match between VetIMPRESS and APHIS.

This form must be printed and completed with ink signatures.

Steps to enable APHIS permission using a prefilled form:
  1. Download the prefilled Farmer Authorisation Form
  2. Pass page one to the farmer to complete and return to the practice, ensuring that they are aware of the following:                                      
    • The Holding Number and Customer ID must be recorded. IF the farmer requires assistance in obtaining their Customer ID, please refer to the appropriate DAERA office, to be reached via the contact information at the bottom of this document. This ID usually starts with a 1 or 2. (Please note also, that Business ID was previously called IACS number or Client ID. This ID usually starts with 6 and it's preferable they supply this also)
    • The Agent is to be recorded as 'Farmvet Systems Ltd'.        
    • Signature(s) must be the same person(s) to which the Customer and/or Business ID refers.
  3. Practice to add the date on page two.
  4. Practice to send the original completed form (pages one and two) in hard copy to the appropriate DAERA direct office (see address list on page two of the Farmer Authorisation Form)                                     
  5. Once DAERA has added the herd to our list of accessible herds, the farmer’s cattle records will be available in VetIMPRESS.
